At the Perth Oral Medicine and Dental Sleep Centre we welcome all new patients. Our practice is limited to the treatment of Orofacial Pain, Oral Diseases and Disorders and Oral Appliance Therapy for Snoring and Obstructive Sleep Apnoea.
If you would like to consult our Oral Medicine Specialists, you are required to obtain a current referral from your Medical or Dental Practitioner.
Please arrive 20 minutes prior to your scheduled appointment to complete important patient history forms. If you wish to complete the patient history forms at home and bring it with you to the appointment, please call the Perth Oral Medicine and Dental Sleep Centre and you will be advised on which forms to download and complete. Please remember to bring the forms with you to your appointment.
If you are unable to attend your appointment, please inform us immediately. As we are a busy practice with many patients requiring prompt attention, we would appreciate your consideration for the other patients. Missed appointments and cancellations within 24 hours may result in a fee.
Unlike medical services, Medicare does not provide a rebate for most dental services. However, your Private Health Insurance Fund may provide a rebate. The extent of your cover depends on the type of policy you have selected, so we are unable to advise on the rebate that you will receive. We recommend that you contact your Private Health Insurance Fund prior to your appointment to determine your level of cover.
We are often asked if we are a "preferred provider" for any Private Health Insurance Fund. Being a "preferred provider" requires contractually committing to a particular Private Health Insurance Fund, and working to a fee schedule and any associated constraints determined by that Fund. The fee schedule may limit the time, care and services available to the patient, and may pre-determine the quality and type of materials used. We do not have a contractual relationship with any Private Health Insurance Fund. At the Perth Oral Medicine and Dental Sleep Centre, we tailor your treatment and care to your needs to ensure uncompromised service.
Full payment for all consultations is required on the day and may be paid by cash, cheque, Eftpos or credit card (Visa, Mastercard and American Express).
